- Description: BACKGROUND Despite the adverse outcomes associated with traditional male circumcision, the practice remains prevalent, especially in the Eastern Cape, South, Africa. This study seeks to assess the practices of traditional surgeons and nurses in relation to the prevention and control of infections and their understanding of human physiological mechanisms during circumcision processes. METHOD This study has adopted a qualitative design, which involved conducting 115 semistructured interviews among traditional surgeons, traditional nurses and traditionally circumcised men, and one focus group discussion among traditional nurses. The data generated were transcribed and subjected to thematic content analysis. RESULTS The analysis revealed that both traditional surgeons and nurses demonstrated both poor aseptic techniques and a lack of knowledge of how the human body functions. Their lack of knowledge of basic human physiology meant that they trivialised sepsis in the penile wound. In addition, the seclusion lodges for circumcision and initiates living were unclean and uninhabitable. CONCLUSION The poor aseptic techniques of traditional surgeons and nurses, as well as the uncleanliness of their environment during traditional male circumcision procedures, could expose initiates to infections and morbidity. Environmental health officers should regularly supervise traditional surgeons and nurses in order to prevent the adverse health outcomes associated with the traditional male circumcision practice.

- Description: Background: HIV/AIDS is a major global public health problem. The higher education sector too, is affected by the scourge of HIV/AIDS. Universities are considered to be high- risks areas for HIV infection because many students indulged in high risky behaviour link to HIV infection; therefore developing ways to prevent students for acquiring HIV and as well reducing HIV prevalence rates through training and teaching about HIV/AIDS will contribute to change in behaviour of students. Anecdotal evidence suggests that the majority of the students at Fort Hare lack the general awareness and education in HIV/AIDS due to lack of information coupled with their general disadvantaged background with the ultimate result being threatened by high infection rates. The University of Fort Hare (UFH), in a bid to capacitate the academic staff in respect of the laudable initiative to integrate HIV/AIDS in curricula, decided to embark on a pilot study that is anticipated to form fundamental strategies to sustainable HIV/AIDS curricula integration across all faculties, at least by the end of 2030. The university has planned to engage in the curriculum integration process by implementing a two year cycle pilot study during the grant period. Aim: The main aim of the study was to assess the challenges faced by students and educators concerning the integration and implementation of HIV/AIDS curriculum at the University of Fort Hare. Methods: The study adopted a quantitative, explorative descriptive design to examine the challenges associated with the Integrated HIV/AIDS curriculum at the University of Fort Hare. The target population was the educators and students who had training and teaching in the Integrated HIV/AIDS curriculum. A purposive sampling was used to select 487 students and 23 educators from across three departments: Nursing Science, LKA (Life, Knowledge and Action) and Law involved in the HIV/AIDS Curriculum Integration programme at the university. A self-design questionnaire was used for data collection focusing on the demographic profiles of the participants, the teaching strategies and methodologies adopted in the Integrated HIV/AIDS curriculum, the usefulness and relevance of the newly integrated HIV/AIDS curriculum content in addressing the needs of the students and the community, and the challenges faced by the educators and students in the Integrated HIV/AIDS curriculum. Ethical approved was obtained from the Research Ethics Committee of the University of Fort Hare. Permission was obtained from the participants through written consent forms. The data was analysed using descriptive statistics (frequency counts, percentage, means and standard deviations (SD)). The Chi-square test was used to examine the relationship between the variables of interest across the departments. A p-value of 0.05 was set for statistical significant testing. Results: The majority of the participants have adequate knowledge of HIV transmission, as 93.2 percent indicated HIV can be contracted through unprotected sex, sharing of needles (92.1 percent), receiving a blood transfusion that has not been screened for HIV (83.9 percent), sharing razor blades that has not been disinfected (83.0 percent), and having more than one sexual partner (86 percent). Age, gender, race and department were significantly associated with knowledge of route of HIV transmission. The present study indicated that the participants experienced several challenges during the teaching of the Integrated HIV/AIDS curriculum. Time allocated was the biggest challenge (58.3 percent), followed by undated materials, and curriculum overloading and teaching. The study indicated that majority of the students would like to compile a portfolio of evidence in the teaching of HIV, followed by creative arts, DVD/movies, and internet research. The majority of the lecturers preferred to teach HIV/AIDS through expressive arts, followed by role play, and formal lectures. The majority of the participants stated that HIV/AIDS information would be useful to them to apply in their communities and in their profession (97 percent), emphasizing that such knowledge and information will empower them to participate in roadshows and voluntary work at HIV/AIDS Shelters. The findings of this study pertaining to the challenges faced by educators during the implementation of the Integrated HIV/AIDS curriculum revealed that the majority of the participants stated that the HIV information was not updated, heavy workload, and adequate training. The lecturers expressed that they would prefer to teach HIV/AIDS curriculum by expressive art through drawing and creating art that expresses HIV and the relevance in the students’ community. Conclusions: Majority of the students received teaching of HIV as part of a course, and HIV teaching as an integrated component in a subject, while few received it as a stand-alone subject offered by a lecturer/facilitator. The present study indicated that the participants experienced several challenges during the teaching of the Integrated HIV/AIDS curriculum. Time allocated was the biggest challenge (58.3percent), followed by undated materials, and curriculum overloading and teaching. The majority of students preferred HIV/AIDS to be taught by their peers experience in HIV, by face-to face by a person who is HIV/AIDS infected or affected. The majority of the lecturers preferred to teach HIV/AIDS through expressive arts, followed by role play, and formal lectures. The majority of the participants affirmed the relevancy of HIV information taught. They further indicated that the information and knowledge gain would able them to make informed decisions on sexual behaviour. The majority of the participants stated that the HIV information was not updated, heavy workload, and adequate training. Most of the educators used formal lectures, followed by compilation of a portfolio of evidence, storytelling, DVD and movies, and experiential learning in hospitals and clinics in teaching HIV/AIDS integrated curriculum. The lecturers expressed that they would prefer to teach HIV/AIDS curriculum by expressive art through drawing and creating art that expresses HIV and the relevance in the students’ community. It is recommended that student’s involvement in the decision making processes of the integrated HIV/AIDS curriculum at the University of Fort Hare would make the programme more successful. HIV/AIDS resource materials need to be updated regularly and relevant to provide educators with specialized knowledge to be able to teach HIV/AIDS.

Factors contributing to non-adherence in HIV positive patients on antiretroviral treatment in primary health care facilities, East London, Eastern Cape

- Description: Aim: South Africa is faced with the major challenge of ensuring and sustaining the quality of services, including preventing and managing side- effects, and improving drug adherence, which are all critical, for the success of a comprehensive HIV programme. The aim of this study was to examine the underlying contributing factors to antiretroviral treatment adherence among HIV positive patients attending primary health care facilities in East London. Methods: This was a descriptive cross-sectional study involving 371 HIV positive patients on antiretroviral treatment in xx primary health care facilities in East London, South Africa. The study utilized a self- designed structured questionnaire with both open- ended and closed ended questions, divided into socio-demographic, socio- economic, level of understanding and knowledge on HIV, quality of care and waiting times. Results: About 64 percent of the participants had a good knowledge of HIV, 25 percent had an average knowledge with few participants having poor knowledge of HIV (11 percent). Majority (92.2 percent) were counselled before starting ARVs treatment. Majority of participants (70.7 percent) do have money for transport to come to the clinic, while 53.1 percent experienced side effects. The majority (70 percent) of the participants were non-adherents. Concerning viral load distribution, 52 percent were undetectable, 26 percent unsuppressed and 22 percent suppressed. Marital status (married) (p=0.005), having no formal education (p=0.035), being Christian (p=0.007), alcohol consumption (p= 0.021) and viral load suppression had significant association with non-adherence to ARV treatment among participants. After adjusting for confounders, only non-Christians and unsuppressed viral loads were the independent predictors of non-adherence. Conclusion: The main contributing factors to ART non-adherence among participants on ARVs were marital status, level of education, religion, and Christians were more adherent to the treatment than non-Christians. Alcohol consumption also contributed to non-adherence and viral load. Adherence counseling should be done on each and every visit to remind the participants on the importance of taking their treatment. These factors need to be addressed to improve adherence to the treatment, decrease the virus in the blood (viral load) and improve their health in general.

- Description: BACKGROUND: Diabetes mellitus (DM) is one of the problematic non-communicable diseases and is a major global public health challenge which affects more than 300 million individuals globally. It is predicted that by the year 2025, approximately 12.3 million South Africans will be receiving treatment for chronic conditions; included in that number will be those living with HIV and diabetes. An individual’s health behaviour is mainly determined by one’s demographic factors such as social, economic and cultural background. These influence people living with type 2 diabetes mellitus and have an impact on their treatment adherence. OBJECTIVES: The purpose of the study was to examine the economic, psycho-social and cultural factors influencing adherence to treatment amongst patients with type 2 diabetes mellitus in order to inform public health policy concerning the treatment and management of diabetic patients. METHODOLOGY: This was a cross-sectional, descriptive quantitative study involving 300 purposively selected clients attending health facilities in Intsika Yethu sub district of Chris Hani, Eastern Cape Province. A self-designed questionnaire was used for data collection. The questionnaire focuses on demographic information (age, gender, marital status, level of education, employment status, number of dependents, duration of taking anti-diabetic medication and existence of other chronic conditions) and the knowledge and attitude of diabetic clients regarding anti-diabetic treatment adherence. Other sections of the questionnaire solicit information on economic, psychosocial and cultural factors affecting adherence to diabetic treatment. The questionnaire was tested in a pilot study in order to identify unclear questions, so that they can be rectified before the main study was conducted. FINDINGS: Based on the findings from the study, a total of 177 (59.0percnt) of the diabetic clients had hypertension and that showed the relationship between diabetes and hypertension. On the attitude session, respondents have shown a positive attitude towards treatment. Furthermore, other objectives such as those focusing on economic, cultural and psychosocial factors were found not to be associated with poor adherence to anti-diabetic treatment. According to respondents’ responses, economic factors showed no influence on treatment adherence. Socio-cultural factors indicated that respondents never missed social gatherings and never used traditional medication to treat their diabetes. On the knowledge section which had fourteen questions, most clients seemed to be aware of the complications of diabetes and the benefits of adhering to anti-diabetic treatment because the lowest score was 50percnt. DATA ANALYSIS: The data was captured in Microsoft Excel (MSEXCEL) for onward processing. Data was analysed using data analysis and statistical software version 14. (STATA). Frequencies and percentages were used for describing the categorical variables while mean and standard deviation were used for describing age, number of dependents, duration of illness and knowledge. An independent t-test was used to determine the differences between males and females across the variables. A p-value of 0.05 was of statistical significance. CONCLUSION: Economic, psychosocial and socio-cultural factors were not found to have impacted on adherence to treatment. These findings should not be generalised as the study was done on a small sample; further research and more studies need to be conducted.

- Description: Background: Non-communicable diseases are a growing public health concern and its prevalence is escalating exponentially, with a high frequency of morbidity, premature mortality, disability and loss of productivity. Since health education has become an important part of medical care, student nurses are potentially well suited to assist with the combat of these diseases. Early identification and treatment of modifiable risk factors among young adults would reduce the risk of developing cardio-metabolic diseases, thus, promoting health, prolonging life as well as saving costs related to healthcare. Objective: This study aimed to investigate the prevalence of cardio-metabolic risk factors among student nurses. Method: This was a descriptive cross sectional study, involving a convenience sample of 228 students during the educational year of 2017-2018 at Lilitha Collegeof Nursing in East London, South Africa. The WHO STEPwise standardized questionnaire was used to collect information on socio-demographic data (age, gender, year of study) and behavioural characteristics (smoking, alcohol consumption, physical inactivity, and dietary intake) of the participants. Anthropometric screening included measurement of height, weight and waist circumference. The biochemical screening included measurement of blood pressure and determination of fasting blood glucose concentration. Only students with complete data set were included in the data analysis. Statistical Package for Social Science (SPSS) version 23.0 was employed for descriptive and inferential analysis. Results: About half (41.7 percent) of the study participants were between the ages of 21 to 25 years and majority were females (67.5 percent).The prevalence of overweight and obesity were found to be 33 percent and 44 percent respectively. The prevalence of overweight was higher in males (44.6 percent), while females (53.9 percent) had higher prevalence of obesity. Female gender and increasing age are the only independent predictors of obesity. Results on blood pressure measurement showed that 25 percent of the participants had elevated blood pressure, and 22 percent were on stage 1 of hypertension while 2 percent were on stage 2. According to Joint National Committee on hypertension (JNC)’s criteria for defining hypertension, 44 percent of the participants had pre hypertension and 11percent had hypertension, diabetes and vegetables consumption were statistically associated with hypertension. The prevalence of pre-diabetes and diabetes was 6 percent and 7 percent respectively. Only abdominal obesity measured by waistto- hip ratio had a significant relationship with diabetes. Just 4.8 percent of the participants smoked daily and 33.3 percent consumed alcohol. Regarding physical activity, males were more active (60.8 percent) compared to females (54.8 percent). Conclusion: A high burden of overweight, obesity, and physical inactivity was observed among student nurses in East London. Screening for cardio-metabolic risk factors (CMRFs) among nursing students and implementation of intervention programs are recommended. Programs to raise awareness about CMRFs, encourage young adult students to adopt a healthy dietary behaviour and promote physical exercise should be initiated.

Quality care is the main focus of the Department of Health in the Republic of South Africa. The researcher believes that professional nurses form the backbone of the whole health care fraternity; hence they are central to the study of quality care dimensions. The aim of the study was to determine and describe the attitudes of professional nurses towards the dimensions of quality care in the community health centres of the KingSabatha Dalindyebo sub-district. A quantitative descriptive design was employed to meet the objectives of the study. The non-probability purposive sampling method was used to select participants to provide information on the attitudes of professional nurses towards the dimensions of quality care in the community health centres of the King Sabatha Dalindyebo sub-district, Eastern Cape. From the target of 84 professional nurses who were employed at the five community health centres of King Sabatha Dalindyebo sub-district of the O.R. Tambo District, Eastern Cape Province, 70 (83,3percent) professional nursesresponded to the questionnaires. A self-administered questionnaire written in simple English was used to collect data from the professional nurses regarding their attitudes towards dimensions of quality care. The researcher adhered to the ethical standards of nursing research as described by the Democratic Nursing Organisationof South Africa regarding anonymity, confidentiality, privacy and quality research, according to which the names of professional nurses were not recorded on the questionnaires. The data were analysed using the Statistical Package for the Social Sciences (SPSS) computer software. The distribution of the results has been presented in tables and a bar graph. The results of the study revealed that the professional nurses of these community health centres had positive attitudes towards the dimensions of quality care.

- Description: In nursing education, experiential learning is an important part of the curriculum and accounts for almost half of the requirements for the nursing programmes. A positive relationship with and collaboration between the training institutions and clinical placement facilities are vital for student achievement. Nurse educators are also expected to involve themselves in clinical accompaniment to bridge the theory-practice gap. Student nurses placed in midwifery clinical learning environments experience difficulty in meeting their experiential learning outcomes and programme requirements. Due to the overcrowded clinical facilities, they compete for the limited learning opportunities and resources which make the integration of theory and practice difficult. Therefore, this leads to delay in the commencement of community service by some of the nursing students as they do not graduate because of not achieving midwifery clinical outcomes and programme requirements. The purpose of the study is to assess factors affecting experiential learning for midwifery students at the public college of nursing in the Eastern Cape and the main objectives of the study were to: • Identify the challenges affecting the quality of learning in the clinical learning environment. • Determine the nature of supervision for experiential learning of midwifery students. • Determine the role of a nurse educator in the experiential learning of student nurses in midwifery clinical learning environment. Methods and analysis A quantitative, descriptive survey was conducted by making use of the Clinical Learning Environment, Supervision and Nurse Teacher (CLES+T) evaluation scale. The CLES+T is a reliable and valid evaluation scale for the gathering of information on the clinical learning environment and supervision of student nurses. The CLES+T evaluation scale was completed by 115 student nurses within the selected sites. The CLES+T evaluation scale is subdivided into three main sections with additional sub-sections: (1) the learning environment (2) the supervisory relationship and (3) the role of the nurse teacher (lecturer).

- Description: The study seeks to assess the need for developing a post graduate child nursing science programme with a problem based learning approach at the University of Fort Hare East London Campus. The aim is to the aim of the study was to conduct a needs assessment in preparation for development of a PBL curriculum so as to produce reflective PNSs. Literature exposed the high mortality rates of children and the competency gaps of trained PNSs. A descriptive quantitative design was used to guide the study. The data were collected by means of self-administered survey questionnaires which constituted closed and open ended questions to yield both quantitative and qualitative data. Data were obtained based on demographic information, performance and importance of the PNS activities, competences of PNSs as well as the prevalence of child health problems at the rural, semi-urban and urban hospitals in the ECP. The purposive sampling technique was used to select participants. The sample size was 70 and 38 participants responded. Quantitative and qualitative data were obtained. Quantitative data were analyzed through use of the Statistical Programme for Social Science software (SPSS) Version 9.0.1 and qualitative data were analyzed manually. Quantitative results were presented with pie graphs, tables and frequencies. Four themes emerged, revealing factors prohibiting the service delivery of PNSs, special training needs of PNSs, prevalent health problems in children and personal interventions. The results showed that in essence there is a need to develop the envisaged Problem Based Learning (PBL) curriculum.

Perceptions of adolescents regarding induced abortion in two public hospitals in East London, South Africa

- Description: Adolescent pregnancy is a concern worldwide and has risen at an alarming rate in South African societies. Many teenage pregnancies end up being aborted. Despite the availability of contraceptives free of charge in public institutions, unintended pregnancies among adolescents still persist. These unintended pregnancies result in a high rate of induced abortion, both safe and unsafe. The study was conducted to explore and describe the perceptions of adolescents regarding induced abortion in two public hospitals in East London, South Africa. This was a qualitative, phenomenological research study to explore the perceptions of adolescents regarding the induced abortion. Participants were purposively selected to participate in the study. Data were collected by means of individual interviews using interview guide in order to allow the participants to express their perceptions freely because of the sensitivity of the issue. Data were collected until saturation was reached. Saturation was reached after 24 participants were interviewed by the researcher. Data analysis used Tech’s (1990) steps as outlined by Creswell (2014:198). Themes, categories, and subcategories were identified following the data analysis. All adolescents admitted having consensual unprotected sex. Engagement in early sexual behaviour by the adolescents was evident as their sexual debut was between the ages of fifteen and sixteen years. Adolescents emphasised that they had abortions because they had no choice. Adolescents cited poor-socio economic status as their main reason for having abortions. Some findings were that adolescent men were against abortion and they had to be persuaded by their female counter parts. A recurrent factor in the results was the infrequent use of contraceptives, including condoms by the adolescents. This indicated that adolescents practised unsafe sex, with little or no regard for sexual transmitted diseases including Human Immune Deficiency Virus and Acquired Immune Deficiency Syndrome (HIV and AIDS). The results also indicated poor treatment by the nursing staff at the family planning clinics. This poor treatment resulted in adolescents not attending family planning clinics. This led to adolescents using abortion as the only method of contraception. Shortage of staff was a contributing factor to the poor treatment of adolescents in the family planning clinics. Adolescents cited that contraception at the clinics is not prioritised. The participants suggested that they should be reminded when to come to the clinic for family planning. Perceptions of reasons for adolescents choosing induced abortion ranges from psychosocial to socio-economic reasons. The knowledge of participants of what was happening to them and what they were doing seemed adequate. The perceptions of the adolescents regarding induced abortion were that abortion is wrong, against their morals as they are Christians; they use abortion because they have no choice due to their poor socio-economic status. The strategies to reduce the unintended pregnancy through the proper management of the contraception programmes, including the change in negative attitudes of health care providers in the family planning clinics could yield positive results.

- Description: The South African Nursing Council requires registered nurses to have the necessary knowledge, skills, attitudes and values which will enable them to render an efficient service (Morolong and Chabeli, 2005:39). The South African Education and training system, through its policy of outcomes- based education and training, has tabled competency as a national priority. In support of this, the South African Nursing Council (SANC) requires that the newly qualified registered nurse possess competency on the core functions of a registered nurse. The health system also demands competent nurse practitioners to ensure quality nursing care. Due to the fact that competency is a national priority and a statutory demand, the research question that guided the study is what are the perceptions of unit managers regarding competencies of newly qualified registered nurses in East London Health Care services. The study was aimed at describing the perceptions of unit managers regarding the competencies of newly qualified registered nurses in order to propose the recommendations to improve the competencies of such newly qualified registered nurses and to provide quality patient care. A quantitative descriptive non experimental design was used to evaluate the perceptions of unit managers regarding competencies of newly qualified registered nurses from a public nursing college. The sampling method of probability convenience sampling was used in this study. Data were collected by means of a questionnaire administered to ninety unit managers and only eighty one unit managers participated in the study. Data were analysed using the Statistical Package for the Social Sciences (SPSS) version 22 software. Statistical methods enabled the researcher to reduce, summarize, organise, manipulate, evaluate, interpret and communicate quantitative data. Data were cleaned and coded and spoilt data were checked. Findings from the software were displayed in graphs or tables. Concerning the eighty one unit managers who participated in the study, age and sex matched respondents were selected according to their availability.The results showed that there is agreement that competence of the nurses affects the image, mission and staff utilisation in their units and/or institutions. There was no dominant opinion on the possible association between competency and environmental, physical and technological factors. There was unanimous agreement that newly qualified nurses need constant supervision and in-service training. There was moderate agreement on the use of acquired knowledge and skills by nurses and there was no dominant opinion about how newly qualified nurses tolerate differences and handle misunderstandings and shortcomings of other professions. In conclusion newly qualified registered nurses need coaching, mentoring, n-service training and constant supervision. Recommendations regarding strategies to be used to improve competencies of newly qualified registered nurses included exit evaluations of community service practitioners, stipulating the competency framework of newly qualified nurses and assessment criteria of competency. Mentoring and coaching of newly qualified registered nurses during community service, monthly or quarterly meetings with the community service candidate in order to identify their challenges were also recommended. Motivation strategies in order to boost their morale so that they can deliver quality patient care. The newly qualified registered nurses should hold meetings with the unit managers regarding the challenges the unit managers face when assessing competencies of newly qualified registered nurses.

- Description: Simulation affords the student a safe and supportive environment in which to practise skills repeatedly without causing any harm to a real patient. The Department of Nursing Sciences at the University of Fort Hare has a laboratory that offers lowto medium-fidelity simulation in the form of task trainers and mannequins. These task trainers and mannequins are used in general nursing, and in community and midwifery nursing sciences, for the demonstration of skills, the practising of skills by students and for formative assessments. In midwifery, task trainers are used for vaginal examinations, abdominal palpation breech and vaginal deliveries and the management of post-partum haemorrhage (PPH). Given that in a real-life clinical environment limited opportunities exist for students to practise during an obstetrical emergency, medium-fidelity simulation (MFS) in midwifery ideally positions the student in a practical environment without risking the patient’s safety. Despite the availability of MFS at UFH, its benefit on the clinical readiness of student midwives is not known. The present study was designed to assess the effectiveness of MFS in enhancing the clinical readiness of student midwives at the University of Fort Hare. The main research objective of the study was to explore, describe and analyse the effect of medium-fidelity simulation on student midwives’ confidence, critical thinking ability, communication, satisfaction and team work in an obstetrical clinical emergency environment. This was a qualitative, interpretive, phenomenological analysis designed to explore the student midwives’ lived experiences regarding the effectiveness of mediumfidelity simulation and the effect of these experiences on clinical readiness. The target population was fourth-year Bachelor of nursing student midwives at the University of Fort Hare. Purposive sampling was used to select five student midwives who were team leaders during the management of PPH using MFS. Ethical approval was granted by the University of Fort Hare Ethics Committee. Informed consent was obtained from the participants prior to data collection. Trustworthiness was ensured by observing the principles of transferability, credibility, confirmability and dependability. Data was collected through individual face-to-face interviews and a semi-structured interview guide. All fourth-year student midwives had viewed an on-line video entitled Essential Steps in Management of Obstetrical Emergency(ESMOE) Postpartum Haemorrhage, in which the procedure was demonstrated. The video, sent via Backboard, was presented in such a way that student midwives had the opportunity to watch it repeatedly in order to thoroughly comprehend the demonstrated skill. A semi-structured interview guide was used for data collection. Interviews were conducted in the simulation laboratory. A Samsung smartphone was used to record interviews and a notepad was used to make notes of gestures, such as smiles or other facial expressions. Data was analysed using thematic content analysis applicable to interpretative phenomenological analysis (IPA) studies using the six steps: reading and re-reading; initial noting taking; developing emergent themes; searching for connections across the emergent themes; moving to the next case and lastly, looking for patterns across cases. The major findings indicated that the participants had differing views regarding the concept of clinical readiness; some held that clinical readiness meant being ready or prepared to handle any eventuality in the course of work, others opined that it was a state of being conversant with the policies and procedures in the clinical ward. Participants believed that clinical readiness was parallel to being competent and that midwives should always be ready to anticipate complications that might arise with the patient, endeavouring to analyse and interpret such conditions clinically. Participants expressed the need to see the simulation skill demonstrated repeatedly for thorough understanding of the technique, so that they could work independently in an obstetrical emergency situation. During simulation, participants were confident in delegating duties to team members, affirming that in order to ensure that tasks were carried out, team leaders should receive regular updates from team members.

- Description: The focus of the study was to explore and describe reasons for poor professional behaviour among nursing students at a public nursing college in the Eastern Cape. The objectives of the study were to explore and describe the reasons for poor professional behaviour among nursing students and to describe strategies to improve poor professional behaviour. The study has significance for the college management, staff, nursing students, registered nurses and everyone involved in professional development of nursing students. The population for this study consisted of third-year nursing students following the four-year comprehensive basic course for registration leading to the Diploma in Nursing (General, Community & Psychiatry) and Midwifery at a public nursing college. The research questions were: What do you understand by professional behaviour? What are the reasons for poor professional behaviour among nursing students? What could be done to improve professionalism among college nursing students? A qualitative, explorative, descriptive and contextual design was used as a framework for the study. Permission to conduct the study was obtained from the University of Fort Hare ethics committee and other relevant authorities as indicated in the study. Ethical principles were maintained and informed consent obtained. Data collection was done using focus groups and an audiotape with a purposefully sampling of 30 third-year nursing students at the public college of nursing. Data were analysed using Tesch’s method of analysis for qualitative research. Three major themes emerged: students’ understanding of professional behaviour, reasons for poor professional behaviour and methods to improve poor professional behaviour. It was concluded that poor professional behaviour has an effect on professionalism and it should be addressed and corrected for protecting the professional image. Recommendations: Policies regarding inappropriate professional behaviour at the college should be in place and easily accessible to every staff member and nursing student. Department of Health should conduct reflective courses and seminars on professionalism and these should begin at the same time as the academic programme.

- Description: Although Nurse Educators are aware of the advantages of simulation-based training, some still feel uncomfortable to use technology or lack the motivation to learn how to use the technology. The aging population of nurse educators cause frustration and anxiety. They struggle with how to include these tools, particularly in the light of faculty shortages. Nursing education programmes are increasingly adopting simulation in both undergraduate and graduate curricula. Scoping literature reviews show that nursing practice has changed in recent years, placing demands on nurse educators to utilise different approaches in education. The fact that nurse educators are an aging population needs to be taken into consideration and acknowledge that many of them did not grow up with computers and lag behind in technological skills. The aim of this study was to investigate the perceptions of nurse educators regarding the use of high-fidelity simulation in nursing education at a South African Private Nursing College in order to be able to determine why High-fidelity Simulators (HFS) have not yet been embraced by nurse educators and students. A national survey of nurse educators and clinical training specialists was completed with 128 participants; but only 79 completed the survey. In addition to background information, participants were questioned about their use of simulators. They were asked to complete the Technology Readiness Index. Information was also obtained regarding their perceptions of the use of HFS. Findings included indications that everyone is at the same level as far as technology readiness is concerned; this, however, does not play a large role in the use of HFS. This finding supports the educators’ need for training to adequately prepare them to use simulation equipment. Recommendations for further study include research to determine what other factors play a role in the use of HFS, studies to determine whether the benefits of HFS are superior to other teaching strategies warranting the time and financial commitment. The results of this study can be used as guidelines for other institutions to prepare their teaching staff for the use of HFS.

Utilisation of university health care services by employees of the University of Fort Hare

- Description: Background: University of Fort Hare (UFH) provides health care services to its students and staff; however, whether the health care services are being utilised by the staff is only speculative. Hence, the aim of the study was to determine the level of employees’ awareness regarding availability of University Health Care Services (UHS) and factors responsible for their utilization. wildlife and thereby may constitute a serious risk to public health. Therefore, it is recommended that proper regulation of the use of the compounds and their safe disposal be ensured to protect aquatic resources and the well-being of humans. Methodology: This quantitative descriptive study involved conveniently selected 255 UFH employees who were either permanently employed or on contract employment for two years and above. A structured self-administered questionnaire was used for data collection. The questionnaire solicited information on demographics variables, awareness of employees regarding the existence of university health services, and factors affecting utilization of university health services (UHS) by employees. Data was analysed using descriptive statistics (frequency and percentages) and inferential statistics (logistic regression analysis). Results: -Two hundred and thirty three (91 percent) of sampled UFH employees in both campuses are aware of the UHS. The longer the duration of employment and the older the employee (above 50 years old) the more likely the employees are aware of UHS. However, the elderly employees displayed poor utilization of health care services. Accessibility of UHS (96 percent), satisfaction with UHS (95 percent), listening to others (word of mouth) (61.2 percent) and having a health concern (51 percent) encourage UHS utilization whereas being unaware (8.6 percent) of the UHS remains the barrier for utilisation. Having medical aid (81 percent) prevent those who are aware of UHS from utilising the university health care services. Operating hours (30 percent), staff attitudes (50 percent) and long waiting time (70 percent) with lack of essential medicines (38 percent) prohibit employees from revisiting. Conclusion: The University of Fort Hare’s health care services are well known by almost every employee through “word of the mouth” though they are not completely aware as to which health service programmes are being offered. The utilization of University of Fort Hare’s health care services is positively and adversely influenced by different factors.
