- Description: BACKGROUND: Operating theatres are one of most expensive resources in a hospital. The management of operating theatres is complex due to the conflicting priorities and preferences of all stakeholders. Operating theatres also pose one of the greatest medico legal risks to any hospital. This study was conducted at Frere Hospital in East London to assess the functioning of the operating theatre system. The focus of this study was on Pre-operative categorisation of emergency cases, the utilisation of the World Health Organisation Safety Checklist, compliance to the National Core Standards for Health, the effectiveness of governance and review processes, theatre staffing as well as theatre efficiency indicators (start time/tardiness, utilisation, turn-around time, re-admission to theatre rate, cancellation on day of procedure, slate under/over runs, Anaesthetic time as a percentage of theatre time). OBJECTIVE: The purpose of this study was to promote the adherence to the various operating theatre universal approaches by operating theatre staff with the goal of reducing costs and errors caused by poor quality in operating theatres and improving the overall health system. METHOD: In this research a convergent parallel mixed method research design was applied. The research was non-experimental and descriptive in approach, using a mixed method questionnaire, a structured open ended interview, a structured infrastructure assessment, a structured observation checklist and an audit of patient files for a review of emergency surgical cases. Data was collected during October 2016 and January 2017.The study population consisted of 13 operating rooms across 6 operating theatre suites, surgical management staff as well as clinical and nursing staff in the operating theatres at Frere Hospital in East London. RESULTS: The study showed opportunities for improvement in the overall management of the operating theatres to ensure efficient quality care; the data from the observations, questionnaires, interviews as well as the analysis of the efficiency indicators consistently showed that an effective process of operating theatre governance and performance is non-existent, the Pre- operative categorisation of emergency cases is inconsistently applied, the compliance to the principles of the World Health Organisation Safety Checklist Is very low, theatre nursing staff levels per case is low, staff are also demotivated and theatre efficiency indicators require improvement. During this study, the Paediatric theatre suite appeared to be well run and compliant with opportunity to increase theatre utilisation, the casualty theatre is underutilised and has potential to be utilised as an enabler to improve the efficiency of other theatres. The main theatre and endoscopy suites have opportunities to improve its compliance to universal operating theatre quality and efficiency standards, the Orthopaedics and Obstetrics theatre suites, however, require urgent improvement focus. DISCUSSION: According to the findings, a structured operating theatre governance and performance review processes should be implemented with the intention of ensuring consistency across the management of the 6 operating theatre suites in the hospital. A review of staffing allocation and relative task allocation should be conducted. Interventions should be implemented to increase utilisation rates, improve the pre-operative categorisation of emergency cases, improve the compliance to the principles of the World Health Organisation Safety Checklist, reduce case start time variance/tardiness, reduce cancellation on day of procedure, increase anaesthetic time as a percentage of theatre time by introducing pre- anaesthetic induction prior to the patient being wheeled into the operating room.

To deal with the determination and re-determination of municipal boundaries the Municipal Demarcation Board (MBD) was established in 1998. The study used secondary data to achieve its objectives. - Description: The study used qualitative methodology to investigate and evaluate the implementation of leadership development strategic plan in the Eastern Cape Provincial Department of Education. The study sought to establish what does the Department‘s policy of leadership development entail to Human Resource Management and Development and what challenges are there in the implementation of the policy and what interventions are necessary to improve the implementation of the leadership policy. The study established that given the need for a transformed public service in line with the post-apartheid democratic dispensation, fundamental reforms to the administration, governance and the funding of education in general had to be given maximum attention hence the need for the adoption and implementation of the leadership development strategic plan in the Provincial Department of Education of the Eastern Cape. The study revealed that the nature of challenges that the department is facing speaks a lot the need for leadership development at the provincial level. However, the implementation of the strategic plan is marred by the challenges that include resources constraints and lack of funding among others. The study therefore came up with some recommendation that can help to improve the implementation of the leadership development plan in the public service sector especially as it relates to the Eastern Cape Provincial Department of Education.

- Description: A clear vision and understanding of the leadership roles by the leaders of the municipality under study are questionable, the low rate of the public service delivery and the lack of accountability by municipality officials also rises some concerns. The aim of this research is therefore to establish a level of understanding of the transformational leadership style in enhancing public service delivery. The study used a qualitative research design that involves a desktop research. Data was collected through the review of available published or existing documents. Data analysis included using documents analysis which consist of the existing information reviewed from literature or document containing information about various municipalities issues, journals and books as well as reviewed of various data analysis done in relation to an exploration of the use of transformational leadership style in enhancing public service delivery with the aim of analysing the organisation structure of the municipality. The findings from previous literature revealed that there are constraints faced in delivering services. The study confirmed some of the municipal leaders’ lack skills, competencies and a lack of clear understand of transformational leadership style as well as training. The findings also identified the search for solutions to the hurdles or challenges faced by the municipal leadership both political and administrative in Buffalo City Metropolitan Municipality by providing an insight for further research regarding the institutional weakness and policy issues in this part of the world. The implications of the findings are that unless the challenges are resolved, municipalities will continue to experience poor service delivery, poor economic growth and development. Recommendations to overcome these are suggested.

The gaps between approved budgets and the realisation of policy and development goals stand among key governance challenges in many developing countries. Supreme Audit Institutions (SAIs) play an important role in holding governments to account. However, many SAIs including AGSA face serious challenges when trying to evaluate the expenditures and performance of government agencies. Therefore this study suggests that more resources should be invested in the office of the AG so as to allow it to carry out its duties without impediments. Good governance and public finance accountability is becoming increasingly important in the public sector. One means of effecting good governance and accountability is through auditing. For this reason the Office of the Auditor-General through the Constitution of the Republic of South Africa 1996 (Act 108 of 1996 section 216 (1)) was established to facilitate and encourage good governance and effective accountability through auditing.

An assessment of the role of Councillors in their oversight mandate: a case of the Chris Hani district municipality: 2011-2013

- Description: According to the Constitution of the Republic of South Africa, Act 108 of 1996, Local Government is the closest sphere of government to the people and thus regarded as the service delivery mechanism of government. In the same context municipal councils are accorded a legal status and authority of a deliberative legislative body. This is considered critical for the municipal council to establish appropriate structures, processes and systems for effective oversight, particularly, oversight of budget execution and administrative processes. However, indications are that, the current legal and institutional measures need to be strengthened to enable municipal council to exercise oversight of the budget execution. In this study, the researcher conducted extensive literature on the roles and responsibilities of Councillors as prescribed in the Local Government: Municipal Systems Act and Municipal Structures Act respectively. An extensive review of the relevant local government legal provisions highlighted the authority of the municipal council, internal rules, reporting processes, council committees and the research capacity, as key aspects of the institutional measures required for oversight of budget execution. Equally, the review accentuated the ability and willingness of non-executive Councillors to exercise oversight in the affairs of the Municipality. A total of 20 research sample was selected for this research. Research participants were given research questionnaires for them to complete. This was completed and returned to the researcher for analysis. Key findings of the research was that, the municipal council must use its legal authority to reinforce mechanisms for effective oversight of budget execution. In addition, the municipal council needs to provide an enabling leadership and governance environment that encourages and support non-executive Councillors to actively engage in oversight processes and activities of the municipality. Further to the findings of this research, it was strongly recommended the further research in order to unearth the mechanisms that can further enable the Councillors to ensure optimum utilisation of resources.

- Description: According to the Constitution of the Republic of South Africa, Act 108 of 1996, Local Government is the closest sphere of government to the people and thus regarded as the service delivery mechanism of government. In the same context municipal councils are accorded a legal status and authority of a deliberative legislative body. This is considered critical for the municipal council to establish appropriate structures, processes and systems for effective oversight, particularly, oversight of budget execution and administrative processes. However, indications are that, the current legal and institutional measures need to be strengthened to enable municipal council to exercise oversight of the budget execution. In this study, the researcher conducted extensive literature on the roles and responsibilities of Councillors as prescribed in the Local Government: Municipal Systems Act and Municipal Structures Act respectively. An extensive review of the relevant local government legal provisions highlighted the authority of the municipal council, internal rules, reporting processes, council committees and the research capacity, as key aspects of the institutional measures required for oversight of budget execution. Equally, the review accentuated the ability and willingness of non-executive Councillors to exercise oversight in the affairs of the Municipality. A total of 20 research sample was selected for this research. Research participants were given research questionnaires for them to complete. This was completed and returned to the researcher for analysis. Key findings of the research was that, the municipal council must use its legal authority to reinforce mechanisms for effective oversight of budget execution. In addition, the municipal council needs to provide an enabling leadership and governance environment that encourages and support non-executive Councillors to actively engage in oversight processes and activities of the municipality. Further to the findings of this research, it was strongly recommended the further research in order to unearth the mechanisms that can further enable the Councillors to ensure optimum utilisation of resources.

- Description: Performance management systems help align individual goals and objectives with those of the organisation. The system engages employees and thereby directs them toward achieving the strategic goals of the organisation. The purpose of this research study was to conduct a critical evaluation on the management and implementation of Performance management and development System in the Department of Local Government and Traditional Affairs. Despite the importance of performance management, most organisations find it difficult to implement, manage and sustain performance management systems and processes effectively. It is therefore crucial to ensure adequate planning, evaluation and training is done that will support a sustainable process. Given the strategic role played by the SMS, especially the management of Performance Agreements in the realization of government programmes, it is important that departmental officials make efforts to ensure compliance with its provisions. This study has shown that officials have not been accurate enough in their implementation of the PMDS. The study reveals that there is no staff development programme in place that would help develop staff members to grow in the different fields of their jobs. This is a sound reason why staff responded that they are not happy with the current system. It would appear that management does not devote a great deal of time to staff development. The focus of the PMDS needs to be changed from an output-focused system to a management development system, where there is an increased focus on the development of competencies, compared to the current focus on rewarding only output.

The study is about: The assessment of relationship between the traditional leaders and ward Councillors and its impact on service delivery: A case of Mnquma Local Municipality. This municipality consists of 61 wards and 41 ward Councillors. The purpose of this study was to investigate what causes tensions in the relationship between traditional leaders and ward Councillors and also how can it be managed and or be improved as it affects production in service delivery and development. Furthermore, the study seeks to find ways through which the relationship between traditional leaders and ward Councillors could be managed to harmonize the relationship between them in order to improve service delivery in Mnquma Local Municipality. Most municipalities which are based in rural areas in South Africa are faced with a lot of challenges which has left development staggering and thus in long run inefficiency in service delivery is caused. As a result of poor service delivery Mnquma Municipality is faced with the challenge of instability towards its management, whereby several Mayors have been chased away through protests, as result service delivery has been hampered and delayed. These challenges are the results of the poor relationship between traditional leaders and ward Councillors over land ownership, control and authority of resources for development. The legislations that govern traditional leaders are ambiguous or rather vague when it comes to their roles and functions in development and service delivery in the new democratic government of South Africa and thus cause the traditional leaders to feel that they are undermined by the ward Councillors whom according to traditional leaders feel they that ward Councillors are more recognized than them. It is in this context that this study seeks to investigate the causes of conflict between the traditional leaders and ward Councillors and how the conflict impacts on service delivery and development and also can it resolved to harmonize the situation. The researcher used a qualitative method to collect data, the questionnaires and interviews were conducted to Municipalities, Traditional Leaders, Municipal Managers, Ward Councillors, Ward Committees and the community. Data was collected and analysed using descriptive method. The researcher came out with the following summary of the findings: The management of relationship between traditional leaders and ward Councillors over ownership of projects, control, land distribution and authority is very poor. Traditional leaders felt that the present democratic government has given too much power to the ward Councillors for most development programs. There is no absolute proper consultation of traditional leaders by neither local government officials, municipalities and ward Councillors before any development is done in their areas. Poor recognition of traditional leaders by ward Councillors and other politicians. There is allegation by the community members and traditional leaders that ward Councillors are bias in delivering services, preference is given to the group that belongs to their party of interest. When there is development projects employment opportunity is not given to the local communities of that area. Traditional leaders are not allowed to actively participate in council meetings.

- Description: This study examines the experiences of the school management teams and educators of the Isibane Circuit in the Queenstown District, regarding the implementation of an Integrated Quality Management System (IQMS) policy at school level. Following an agreement reached in 2003 between the South African Education Department and the major teacher organisations in this country, the IQMS was scheduled to be implemented in public schools from 2004. Two major challenges are identified and explained: accountability and development of human resources, as well as contradictions between internal and external evaluations. The study also elaborates on the changes in the educational history from the apartheid system to the democratic system of education. Nine schools participated in the study (two secondary schools and seven primary schools). These schools were purposely selected due to the fact that they had started implementing the IQMS policy, which was the main criterion for selection. Semi-structured interviews were used to gather data from principals, Heads of Departments as well as post-level one educators concerning their experiences of the IQMS implementation. The findings indicated that while the IQMS policy was good on paper, it was extremely difficult to implement. Issues of honesty and trust were at the root of difficulties that made the IQMS implementation challenging. The participants‟ understanding of IQMS was that it was intended for the provision of quality education and development.
