Clinical experiences of third-year student nurses in a public college in the Eastern Cape

- Description: Clinical experience has always been an important part of nursing education. Nursing colleges require students to be placed in clinical areas in order to acquire new knowledge and skills. The purpose of the study was to explore and describe the clinical experiences during their clinical placement of the third-year student nurses at a public college in the Eastern Cape. The objectives of this study were: to explore and describe the experiences of the third-year student of a public nursing college in the Eastern Cape, and to describe strategies for the improvement of student learning during clinical practice. A qualitative, explorative and descriptive research design was used to describe the clinical experiences of these student nurses and the strategies for the improvement of student learning during clinical practice. Data were collected from focus group interviews, using a purposive and convenience sample from one college campus in the Eastern Cape. Tesch’s method of data analysis (in Creswell, 2003) was used to analyse data. Trustworthiness was ensured in this research by using Guba’s model (in Lincoln & Guba, 1985) of qualitative research. Ethical approval was obtained from the University of Fort Hare Research Ethics Committee, the Department of Health, Eastern Cape, the Head of the Nursing College, the Campus Head and the participants and ethical consideration was ensured throughout the research process. Themes and sub-themes emerged from data collected about the clinical experiences of the nurses. The study brought to light the experiences of third-year student nurses during allocation to clinical areas. The findings from this study revealed the need for proper clinical accompaniment by college staff to ensure that clinical learning is taking place. Strategies to improve learning in the clinical areas are described. and recommendations for nursing education and nursing practice are made. If this is applied, it will help the student nurses to be better professionals.

- Description: Basic Antenatal Care (BANC) is a way of training or upgrading the knowledge and skills of all nurses, midwives and doctors involved in antenatal care at the primary health care level so that the minimum care can be provided effectively. This study was conducted to investigate the implementation of the BANC program by midwives in the Mdantsane clinics during February 2009. Methods: A descriptive study design was undertaken targeting midwives providing antenatal care to pregnant women, in 14 clinics of Mdantsane. Data was collected from 25 midwives in the clinics, and from 140 ANC cards of women attending ANC on the day of their visit to the clinic. Results: The majority of midwives providing BANC in Mdantsane clinics were not trained in BANC. There were 10 trained midwives and 15 not yet trained. A total of twenty five midwives were involved in the study. The number of visits according to the BANC schedule was well known by the midwives in the study. The content of the visits was well known for the first visit, but for subsequent visits, the participating midwives could not state exactly what they do on these visits. They perceived BANC as something beneficial for both midwives and pregnant women with 24 of the participating midwives rating BANC as advantageous. In completing an ANC card, the midwives scored between 48 percent and 100 percent. Under examination, they scored between 52 percent and 100 percent. Lastly on interpretation and decision making, they scored between 0 percent and 92 percent. This could have troubling consequences for the health status of the mother and baby. Weaknesses in providing antenatal care identified in the study included participating midwives failing to fill in the last normal menstrual period (LNMP) and the estimated date of delivery (EDD), which was a worrying observation. Plotting of the gestational age at first visit was also not carried out well as only 47 percent of the midwives in the study did this, meaning that there would be a miscalculation of the gestational age thereafter throughout the pregnancies. The body mass index (BMI) was not calculated as the maternal height and weight were not written on the ANC card. This should be completed in order to check the nutritional status of the pregnant woman to help supplement, if malnourished, and educate on diet, if overweight. Only 17 percent of the midwives in the study plotted the foetal presentation. Failure to plot foetal presentation could lead to complications during delivery because women with abnormal presentations could end up delivering in a clinic instead of the hospital.Conclusion: This study showed that even though midwives are implementing BANC among pregnant women, it is not being carried out correctly. Therefore the programme will not be as beneficial as it would be if put into practice correctly. This is highlighted by the lack of knowledge from the untrained midwives regarding the content of care on subsequent visits. Thus there is an urgent need for BANC training to be conducted and monitored at various sites.

The impact of HIV/AIDS on the delivery of maternal health care services of selected primary health care clinics of Peddie in the Eastern Cape Province

- Description: The aim of this descriptive study was to explore the impact of HIV/AIDS on the maternal health programme at selected Primary Health Care (PHC) clinics in the Peddie sub-district, in the Eastern Cape, South Africa. This study showed that the HIV/AIDS epidemic has had major influences on the rendering of maternal health services in this sub-district. An understanding of the impact of HIV/AIDS on the delivery of the maternal health programme in the Peddie Primary Health Clinics will potentially help the District Health Service office, as well as the clinic managers and professional nurses to formulate strategies to strengthen the maternal health programme. This data will also be helpful when developing plans to cope with the heavy work-load of the clinics offering maternal health services, and to address the attitudes and concerns of the professional nurses working in the PHC clinics in this area. Thirty professional nurses working at 10 clinics responded to the questionnaires which elicited questions pertaining to the impact of HIV/AIDS on the rendering of their services. As part of the data gathering procedures, standardised notice boards at the PHC clinics were used to document the services rendered. The numbers of ANC clients attended to each of the clinics per week were collected in routine attendance tick registers used in the clinics for recording and monitoring statistics in the maternal health programme. The data was analysed to examine the possible impact of HIV/AIDS on maternal health services rendered by registered nurses/midwives working in the PHC clinics. The registered nurses who responded to the questionnaires (Annexure H) made several suggestions. These included the following: (i) encouraged more training (11 nurses), (ii) addition of more staff members, (iii) to have specialized staff for management of maternal HIV. Because of the study limitations such as the sample size of 10 clinics with a total of 30 nurses who were studied, the study may not be generalised to the whole of the Eastern Cape Province. The study however did provide baseline information on the impact of HIV/AIDS on the maternal health programme. The study findings were that HIV/AIDS has had an impact on the delivery of the maternal health programme due to the extra services brought about by the response to the HIV/AIDS pandemic. This study will contribute to a better understanding of the impact of HIV/AIDS on the maternal health programme at the PHC level in Peddie.
