Towards an understanding of Amayeza esiXhosa stores (African chemists): how they operate, and the services they offer in the Eastern Cape

- Description: In medical anthropology there has been a tendency to dichotomize western biomedical . healtb services, on the one hand, and traditional health care practices on the other. Much attention has been focused on the comparison between these two approaches in the hope that they might be reconciled. The problem with this approach is twofold. In the first place, it has not always acknowledged the local, historic~1, political and economic contexts in which different approaches to health care have evolved and in the second place, health care services which belong to neither the western nor traditional healing spheres and which are driven by commercial interests have been almost completely neglected because they fall outside of the basic dichotomy. Amayeza stores have been a feature of South African towns and cities for many years. They mayor may not be run by Africans, but their clientele is almost exclusively African in this region. They deal in a bewildering variety of products and remedies, from untreated herbal and animal products to pharmaceuticals specially prepared for the African market, to Dutch and Indian Remedies. These stores both reflect transfonnations in indigenous perceptions of health care and, by virtue of the choices they offer, generate change. In this empirical study three stores in the Eastern Cape are selected for detailed study - two in King William's Town, the regional capital, and one in the small town of Peddie. The approach is holistic, emphasizing the social, political and economic context, the business histories and running of each shop, and, in particular, the perceptions and choices of a sample of the customers in each case. The success of the amayeza phenomenon derives from its eclecticism and syncretism. These stores impose neither a western nor a traditional model of health care on their clients, but offer them a range of choices that reflects the complex multicultural history of their own South African society.

- Description: The present study specifically focussed on Faith healing as an indigenous healing system and its influence on the diagnosis and treatment of mental illnesses in the Eastern Cape province of South Africa. The study aimed to provide a descriptive overview of Faith healers’ perspectives on the diagnosis and treatment of mental illnesses in the Eastern Cape, as well as to compare conclusions reached with other categories of indigenous healers. The inter-category comparisons on a broader level allowed for a further comparative discourse with the mainstream western medical psychiatric view of mental illness. Therefore, the scope of this study does not include in-depth analyses of findings, but rather the generation of themes for comparative discussions. While there exists vast literature on the diagnostic and treatment perspectives of the other two categories of indigenous healing systems (traditional healers and herbalists), a limited number of studies have been focussed on Faith healing as an indigenous mode of healing. The present study attempted to address this gap in the literature in an effort to promote future collaborative work across all viewpoints, in the management of mental illnesses. This study, grounded in qualitative research, utilized thematic analysis as its theoretical framework. Non probability judgmental sampling was used to secure self-identifying Faith healers, where conclusions from them were drawn from data collected, using in depth semi-structured interviews and observation. The main findings of the study indicated that Faith healers’ perspectives on the diagnosis and treatment of mental illnesses are based mainly on indigenous cultural theories. Furthermore, that collaboration with other viewpoints is hampered by animosity, feelings of distrust and the fear of appearing inferior.

- Description: Over the past 25 years, there has been a resurgence of worldwide scientific research in the fields of ethnopharmacology. The Western world has acknowledged the continued use of traditional medicines by the majority of third world countries, and the need for novel drug development. Hence, much of the pharmaceutical research in recent years has focused on the ethnobotanical approach to drug discovery (Light et al., 2005). In South Africa, as in most developing parts of the world, traditional herbal medicine still forms the backbone of rural healthcare. The government health services in South Africa provide only western medical care although the majority of the population consult traditional healers for some or all of their healthcare needs (McGaw et al., 2005). Medicinal plants like Harpephyllum caffrum are used as blood purifiers or emetics (Watt and Breyer-Brandwijk, 1962), and also for treating acne and eczema. The antimicrobial activity of this plant can be used to treat septicaemia, which is ranked the sixth leading cause of death among neonates and the eighth leading cause of death for infants through the first year of life (Heron, 2007). In this study, the plants investigated for antimicrobial activity were Harpephyllum caffrum, Hermannia cuneifolia, Chironia baccifera, Rhigozum obovatum, Felicia muricata and Pentzia incana. These plants were tested against ATTC (American Type Culture Collection) strains and microorganisms isolated from clinical isolates of patients suffering from septicaemia. The assay methods used included the agar diffusion method using the Mast multipoint inoculator, the microtitre dilution method were used to determine the minimum inhibitory concentration, thin layer chromatography fingerprints accompanied by bioautographic assay were used to detect the inhibition of bacterial growth by active compounds separated from plant extracts and the Ames test was required to assess the possibility of bacterial mutagenesis upon the exposure to plant extracts which can lead to carcinogenicity. In agar diffusion method, extracts of Harpephyllum caffrum inhibited nine strains of Candida albicans, three species of Acinetobacter and four strains of E.faecalis. Extracts of Hermannia cuneifolia inhibited four strains of B.cereus and three strains of Staphylococcus aureus. Extracts of Chironia baccifera inhibited one strain of Acinetobacter and five strains of E.faecalis. Extracts of plants Rhigozum obovatum, Felicia muricata, and Pentzia incana showed no antimicrobial activity. In the microtitre dilution method used to determine the minimum inhibitory concentration (MIC), the results were different from the agar diffusion method. More activity was observed. Extracts of Harpephyllum caffrum inhibited three strains of E.coli, six strains of S.aureus, three species of Acinetobacter and one strain of Klebsiella pneumonia. Extracts of Hermannia cuneifolia inhibited four strains of B.cereus, three strains of S.aureus, two strains of K.oxytoca and one species of Acinetobacter. Extracts of Chironia baccifera inhibited three strains of S.aureus, one strain of MRSA, one species of Acinetobacter and one strain of S.haemolyticus. The MIC values ranged from 0.049 to 6.25mg/ml. Using the thin layer chromatography fingerprints, bioautography showed the presence of various inhibitory chemical compounds. Methanol and acetone extracts of Harpephyllum caffrum, separated very well and showed various inhibition zones on exposure to Candida albicans, Enterococcus faecalis and Staphylococcus aureus. The different inhibition zones were recorded as Rf In the Ames test (Maron and Ames, 1983) the methanol and acetone extracts of Harpephyllum caffrum and Hermannia cuneifolia were negative which means they were devoid of any mutagenic properties. Methanol extracts of Harpephyllum caffrum showed similar results in the Ames assay as reported by Verschaeve and Van Staden (2008). values ranging from 0.25 to 0.95. The zones indicate the different inhibiting chemical compounds present in the plant. Petroleum ether, ethyl acetate, chloroform and formic acid were the solvents used in the assay in the ratio 8:7:5:1, respectively. Establishing the antimicrobial activity of these plants contribute to the systematic scientific investigation of indigenous South African medicinal plants.

- Description: The primary objective of this study was to understand to the role that discourse on traditional medicine plays in the choices that Rhodes University students and staff make regarding who they consult when they are sick. A secondary objective was to explore the participants’ views on the Department of Health’s draft policy on the institutionalisation of African traditional medicine and potential collaboration with the biomedical health care system. This was a qualitative study that used in-depth interviews and questionnaires to collect data, which was interpreted using thematic data analysis. A key finding was that participants were mostly influenced by their families with regard to the health care choices – either biomedical or traditional medicine. And that affordability, accessibility and availability are not important factors in health care choices as suggested by literature. Participants also based their decisions on what they believed would help them, either biomedicine or traditional medicine.

- Description: Pelargonium sidoides DC. (Geraniaceae) root extracts are used in the Eastern Cape Province of South Africa as a traditional medicine for the treatment of respiratory tract and gastro-intestinal infections. Ethanolic extracts are used globally as herbal treatments for bronchitis, asthma and as an immune system booster. Despite documented exploitation of wild populations by illegal harvesters, this species has not been awarded a protected status. The high level of harvest in the years preceding this study prompted this investigation of the prospects for sustainable root harvest through wild harvest and greenhouse cultivation. A novel method was developed for the purification of umckalin, a bioactive constituent in root extracts, such that the root umckalin concentrations of wild and cultivated plants could be quantified by HPLC. As part of the cultivation experiments, the concentration of umckalin in roots was measured for plants across part of the species’ distribution range in the Eastern Cape Province. This survey revealed that root umckalin concentrations were inversely related to the average annual rainfall of the collection site (r² = 0.94, p = 0.007) and directly related to soil pH (r² = 0.97, p = 0.002). Thus, the possibility of inducing high umckalin concentrations in greenhouse-cultivated plants was investigated by subjecting plants to rapid and prolonged water stress treatments. Two leaf applied hormone treatments (cytokinin and gibberellin) and a root competition treatment with a fast growing annual (Conyza albida) were also investigated based on the potential function of umckalin in P. sidoides plants. These five treatments did not significantly affect root umckalin concentrations compared to well-watered controls. The results of further experiments suggested that umckalin production may have been influenced by the geographical origin and genetics of plants rather than environmental variation. Following wild harvest experiments, the regrowth of replanted shoots from which a standard proportion of the root was harvested showed that water availability affected shoot survival but not root regrowth rate. Regrowth rates were low, questioning the viability of wild harvest. In contrast, greenhouse cultivated plants showed ca. six times greater growth rates, supporting the cultivation of roots to supply future market demand.
